Could someone here please help me identify this girl?

She was in an issue of FHM back in 2006.
I believe this was the issue that she appeared in...

I cut her picture out, and threw away the rest of the magazine without getting her name, and would like to know who she is.
I don't have a scanner, so I filmed the photo with a poor quality web camera.
